Direct (Sashimi) lip lift

A direct lip lift is a procedure that optically enlarges the lips and makes the face more attractive. A direct lip lift can be performed on both the upper and lower lips. A direct lip lift is suitable for those clients who want fuller lips and for those who regularly undergo lip filler procedures and would like to achieve a permanent effect. We optically enlarge the lips with this procedure, and often fillers are not necessary any longer. Other suitable candidates are clients who have thinning lip vermillion, as well as clients who have wrinkles above their lips. During a direct lift of the upper lip, the surgeon shortens the philtrum, i.e., the distance between the nose and lips, thus revealing more of the lips´ pink vermillion. The ideal distance between the nose and the lips should be about 1.2-1.5 cm. Some people have a longer philtrum genetically, while others have it lengthened with age. A direct lift of the lower lip can be performed also separately or together with the upper lip.

Description of the surgical procedure

Consultation

An initial consultation is a crucial part of the surgery. The client tells the surgeon the reasons why they decided to undergo the procedure, and the surgeon explains what the client can expect from the surgery, what results are realistic, and advises them on the most suitable procedure for them. During the consultation, the doctor answers all the client’s questions.

Before the procedure

Before each surgery, it is necessary to assess the overall health of each client. With local anesthesia, blood samples can be taken in a laboratory or by a general practitioner or internist.

The procedure

A direct lip lift is performed on an outpatient basis under local anesthesia and takes approximately 45 min to complete. Before the surgery, the client discusses everything with the surgeon once again and then receives premedication. In the operating room, we administer a local anesthetic by injection. This may be slightly uncomfortable, but the injection only takes a moment, and the client feels almost nothing during the subsequent operation. The incision is made on the vermillion border of the upper lip (in the case of the lower lip, on the vermillion border of the lower lip). The wound is closed with a very fine stitch.

After the procedure

Immediately after the procedure, the scar is covered with a plaster, which is left in place until the stitches are removed. The client leaves the clinic after the operation. A 24-hour hotline is available, which the patient can contact if necessary. The stitches are removed on the 9th day after the surgery. It is crucial to follow a rest regimen for at least three days. Swelling and bruising occur individually and usually last for 1-2 weeks. You can return to normal office work after just a few days. Two weeks after the surgery, the client starts doing gentle pressure massage on the scar and begins to lubricate it with silicone cream. The scar is less visible after two weeks and barely noticeable after a few months, but it is important to take care of it. Pressure massage should be performed twice a day for several months. The postoperative care bag that the client receives from us after the operation contains all the necessary medications and creams.

Results and durability of direct lip lift

You will see the difference immediately after the procedure, however, the final result will only be visible approximately 1-2 months after the operation, when the mild swelling subsides. The durability of the results of a direct lip lift depends on age, genetics, and lifestyle. The procedure can also be combined with other rejuvenation procedures in the facial area.

Risks

No surgery is completely risk-free. However, these risks are minimized by a skilled surgeon, adherence to preoperative instructions, and conscientious postoperative care. The client is informed of the most common risks before the surgery.

Frequently asked questions and answers

A bullhorn lip lift is a procedure with a scar that is well hidden under the nose, ideal for slightly shortening the distance and gently enlarging the upper lip. A direct lip lift offers a more noticeable change, but the scar is on the red part of the lip and may be more visible.

The scar after a direct lip lift is on the red part of the upper lip, which means it may be more visible and take longer to heal than a scar under the nose. However, with careful care and the correct procedure, most clients achieve a subtle and inconspicuous scar.

Yes, they can be combined – for example if the client needs to reduce the distance under the nose and at the same time lift the corners of the mouth or adjust the shape of the upper lip. The surgeon will evaluate which combination is suitable with regard to the final effect and minimal visibility of scars. The combination can be planned so that the procedures complement each other and enhance the overall appearance of the lips and smile.
